What states produce the happiest women physicians?
On/Offcall is the weekly dose of information and inspiration that every physician needs.
As part of Offcall’s work to build the world’s largest dataset on physician compensation and workload, we’re highlighting a range of data insights — from physicians’ views on supervising APPs to job satisfaction across metro areas and the specialties reporting the highest and lowest satisfaction. Explore the full collection of past data stories below.
Recently, we investigated the gender pay gap in emergency medicine, and in today’s post, we’re highlighting trends in female physician job satisfaction across some of the most popular states on Offcall. Job satisfaction can vary significantly based on state – especially for female physicians. Variations in family leave benefit requirements, workplace harassment protections, pay transparency, and regional workplace culture can have a particularly significant impact for female physicians.
To measure satisfaction, we asked physicians on Offcall to rate their jobs on a simple 1–5 scale. We then aggregated results by state to see where female physicians tend to feel the happiest, and the results reveal some clear standouts.
Massachusetts, New York, and Illinois reported the highest job satisfaction with mean ratings of 3.50, 3.48, and 3.45. On the other hand, Pennsylvania reported the lowest job satisfaction with a mean of 3.11, followed by Ohio and New Jersey at 3.26, and 3.29 respectively.

Numerical differences might seem small, but across thousands of responses, these patterns point to real differences in day-to-day experience.
